PAYMENT

Associates, Inc. participates in many insurance plans, however the
insurance arena is in a constant state of change. It would be in
your best interest to check with your specific insurance company
to make sure that they have our doctors listed as participating
physicians prior to the time of your appointment.
 When
you come to your appointment you will need to bring your insurance
cards, as well as your referral if your insurance requires this.
(Some insurance referrals are transmitted electronically. If your
primary physician uses this method you may want to ask for the referral
number as a back up.) If you have a co-pay please be prepared to
pay this at the time of the appointment.
We accept cash, check or money order, or credit cards. If your insurance
does not cover office visits we will expect payment at the time
of your visit.
Occasionally
there is a problem with a referral and by the rules of the managed
care insurance companies we cannot proceed with the visit unless
the patient agrees to do one of the following:
- Sign
a waiver of responsibility. We will not consider you responsible
for the bill until 24 hours later, which gives you the chance
to get a proper referral.
- Reschedule
the appointment.
