MAKING AN APPOINTMENT

When you initially call Gastrointestinal Associates you will be asked for some initial information including:

• Name and address
• Date of birth
• Name of primary physician
• Type of insurance plus insurance numbers
• Social Security Number
• Problem bringing you to our practice

You will be given an appointment with one of our doctors. You will be sent forms to complete and bring with you on the day of your appointment. You will be asked to get any medical records that may help with your care. Please bring these with you to your first appointment.

Revisit

When you call your initial information will be reviewed for changes. If it has been a year or more since your last visit you will be sent forms to complete. If you have not been seen in a year or more of if you have a new problem we will want any appropriate medical records brought with you to your visit. If you have had any changes of significance since your last visit to our practice please let our phone operator know.
