FIRST APPOINTMENT

In order for us to better serve your medical needs, we have supplied our Patient Information Form and our Health Questionnaire. Please complete the forms and bring them with you along with your Health Insurance cards to your appointment. As a reminder, if you are a Member of an HMO, you will need a referral from your primary care physician authorizing each visit. If you do not have a referral, we will offer you the option of paying for the visit or rescheduling your appointment.

To provide the best care possible, we ask that you bring with you at the time of your visit:

• Referral
• Insurance card(s)
• Co-pay
• X-ray reports and films, along with any other medical records.

You will be given an appointment with one of our doctors. You will be sent forms to complete and bring with you on the day of your appointment. You will be asked to get any medical records that may help with your care. Please bring these with you to your first appointment.
